What should schools be doing?

  • Ensure there is a policy in place for managing long term medical conditions
  • Complete the Individual Healthcare plan in partnership with the child’s parents/carers and catering staff
  • Have a system in place to identify the requirements of children with medical conditions

Individual healthcare plan template

The best way to make sure a pupil is getting the support they need is to have a plan. Your school should arrange to draw up an individual healthcare plan in partnership with the child’s parent/carers, teachers, catering staff and school leadership. This should highlight the key points in the school day and across the year where adjustment may need to be made to support the child to take part fully in school activities.
